【摘要】 目的 確認2008年5月廣州市番禺區1起嬰兒手足口病死亡的病因。方法 訪談病例發病前后相關知情人, 查閱臨床病志, 對病家開展現場調查和病例主動搜索。采集病例肛拭子、口腔分泌物、鼻腔分泌物、腦脊液, 應用逆轉錄-聚合酶鏈反應(RT - PCR) , 檢測腸道病毒( EV) 的EV71和CA16特異的核酸片段。結果 患兒無外出, 無接觸史, 病家及周圍衛生條件差。5月1日, 患兒發熱, 手、足及口腔出現散在皰疹, 5月5日, 出現抽搐、巴氏癥陽性、雙肺可聞及大量痰鳴音等重癥表現, 死于腦炎和肺水腫。實驗室結果為EV71陽性、Cox A16陰性。結論 該患兒為7EV71感染的重癥手足口病例, 但感染途徑未查明。
【Abstract】 Objective To confirm one case of baby hand, foot and mouth disease causes the death of ways in May 2008 in Panyu District, Guangzhou City. Methods Before and after the onset cases related to insiders was interviewed and medical records were collected. Site investigations and cases of active search were conducted to the case and his households. Acquisition cases anal swabs, oral secretions, nasal secretions, cerebrospinal fluid, reverse transcriptase - poly - merase chain reaction (RT - PCR) , detection of the EV71 and Cox A16 - specific DNA fragment. Results The results in children with no bail, no history of exposure, the case to his households and the surrounding poor sanitation1 children had fever, hand, foot and oral herpes in a casual, in May 1, and a convulsion, Pakistan’s disease positive, lungs can hear the sound - ming and a large quantity of sputum, such as severe performance, in May 5, died of encephalitis and lung Edema1 EV71 positive laboratory results, CoxA16 negative. Conclusion The EV71 infection for children with severe cases of hand, foot and mouth, but did not identify means of infection.
